1. Download and fill the Admission Form
2. At the time of submission of form, attach the following :

A demand draft of Rs. 250/- towards admission form and processing fees in favour
of principal, SHEOPUR INSTITUTE OF PROFESSIONAL STUDIES,SHEOPUR(M.P.)
b. Photograph Pasted on the
application form.
c. One stamp size photograph.
d. Photocopy of Marksheet of 10th
e.
Photocopy of Marksheet of 12th
f. Photocopy of Marksheet
of Graduation
g. Photocopy of domicile certificate
h. Photocopy of caste certificate [ For SC/ST/OBC
students]
i. Photocopy of class certificate
[if applicable]
3. At the time of confirmation of admission ,submit the following:
a. Two Passport Size Photographs
b.
Two Attested photocopies of 10th Marksheet
c. Two
Attested photocopies of 12th Marksheet
d. Two Attested
photocopies of Graduation Marksheet
e. Two Attested
photocopies of domicile certificate
f. Two attested
photocopies of caste certificate [ For SC/ST/OBC students]
g.
Two attested photocopies of class certificate [ if applicable]
h.
Two attested photocopies of income certificate[ For SC/ST/OBC students]
i.
Original School/College Transfer certificate and two photocopies.
j.
Original Gap certificate / Affidavit ( if applicable) and two photocopies.
k. Original Migration certificate ( if applicable) and
two photocopies.
l. Photocopy of enrolment No. of
Last Exam ( if passed from Jiwaji University)
4. Admission will be confirmed only
after the submission of all the above said documents and full fees
payment .
